How do I schedule a VA disability evaluation?

You can schedule an evaluation by filling out the contact form above and selecting 'VA Disability Evaluation' as your reason. A care coordinator will contact you to review your requirements and set a date at one of our DR locations.

Do I need to be enrolled in FMP to receive care?

While we can treat you without current FMP enrollment, we highly recommend it for service-connected conditions. Our staff can assist you with the FMP registration paperwork during your first visit to ensure your care is properly documented.

What documents should I bring to my first appointment?

Please bring a valid photo ID (Passport or Veteran ID), your VA award letter (if applicable), and any recent medical records related to the condition we are treating. Digital copies are also accepted via our secure portal.

Can I use VAFMS services if I am just visiting the DR?

Yes. We provide care for both residents and visiting veterans. If you have an urgent care need related to a service-connected condition while traveling, please contact our clinics directly for immediate scheduling.

Is VAFMS a part of the U.S. Department of Veterans Affairs?

VAFMS is an independent medical provider. We are not a government agency. However, we specialize in VA-aligned healthcare and documentation standards to ensure your medical evidence meets the requirements for U.S. VA claims.
